A Case of Spurious Thrombocytopenia

Platelet estimation is important in the diagnosis and management of both hematologic
and non-hematologic disorders. With automated electronic counters, reproducible and
accurate platelet counts are usually obtained with ease. However erroneous results may
attend automated platelet counting and that thrombocytopenia may be apparent but not
real. We have encountered a case of spurious thrombocytopenia in a 49-year-old male
patient. He was admitted to our hospital due to car accident. His peripheral blood smear
showed marked platelet clumping, however, platelet count measured with electronic
platelet counter was very low (24, 000/§§). Changing anticoagulant from EDTA to
sodium citrate for collection of venous blood, the peripheral blood smear showed even
distribution of platelet and the count was adequate (238, 000/§§).
